The patient is A 28 yr old female, diagnosed with Nodular nontoxic goiter who was classified as ASA 1 preoperatively and underwent total thyroid lobectomy, left and isthmusectomy. She experienced symptoms of acute coronary syndrome three days postoperatively .

Her past medical history was unremarkable except for easy fatigability. Since this was thought of as just one of the symptoms of her primary problem and not associated with other co morbidities, no further work-ups were done. Family, social and environmental history was also unremarkable.
